a) The candidates should apply in prescribed application format attached with this notification and should submit the same by hand in the Drop box placed at their respective SDO office or by ordinary post to their respective SDO
office. Documents to be submitted with the application form:

2. Residence Proof: Voter Card/Adharcard/Ration Card/residence certificate of Panchayat Prodhan (duly counter
signed by Panchayat Secretary)/ residence certificate of Chairman of Municipally (duly counter signed by EO,
Municipality)
3. Academic & Computer Qualification : Self attested copy of Academic & Computer qualification for the post
as mentioned in TABLE-A.
4. Certificate of Caste : Self attested copy of cast certificate received from an appropriate authority
5. Experience Certificate: Self attested copy of Experience certificate from appropriate authority as mentioned in
TABLE.A.
6. Two self-addressed envelope with requisite postage stamp be submitted with the application from.
7. Two pass port size (2.5×3.5cm) photo duly signed by the candidate
